Each plan includes Includes 1 office visit, 1 follow up telehealth visit. Other fees may apply for additional services.*
*Additional fees may apply for services such as labs, imaging, point of service tests, minor procedures, etc.
We do not prescribe pain medication.
We also offer Home visits and Telehealth visits for Non-members (conditions apply).
